OHS Information and Forms
The Office of Hearing Services (OHS) is run by the Australian government. It allows eligible Australian Pensioners and Veterans to access free hearing services, including hearing assessments, hearing aids and hearing rehabilitation. If your patient is fitted with hearing aids under the OHS Scheme they are also able to pay a small yearly fee which then covers their aids for repairs and batteries for that year.
Applying for free Hearing Services through the Office of Hearing Services Program is now very straightforward.
If your patient requires a hearing test and is an Australian Pensioner in receipt of an Australian pension simply complete the one-page OHS Medical Certificate Form OHS Medical Certificate Form.
Once completed and signed by both patient and doctor, you or your patient can call Hearing and Audiology on (08) 9388 8003 and one of our friendly staff will assist your patient in obtaining their voucher for free hearing services. The form can be emailed to firstname.lastname@example.org or faxed to (08) 9388 8009.
